Participates in the overall development, implementation, and growth of diabetes education programs at Cleveland Clinic. Provides diabetes education resources and services to caregivers, patients, and the community.
Provides revenue generating, fiscally responsible diabetes education services for the assigned patient population.
Provides complete and concise documentation of each patient encounter in the electronic medical record.
Supports the training of clinical staff in diabetes care.
Supports diabetes-focused research initiatives.
Plans and provides one-on-one and/or group instruction in all diabetes core content areas to persons with diabetes based on individualized needs assessment
Facilitates active participation by the patient to identify individualized teaching/learning needs
Facilitates the setting of reasonable and obtainable goals by the patient
Provides/maintains communication among diabetes team members regarding status of diabetes education process
Supports ADA program recognition through appropriate education, documentation, and CQI project participation
Participates in community outreach projects
Other duties as assigned.
Bachelor's of Science degree in Nursing or related field or the equivalent in training and experience.
Current state licensure as a Registered Nurse .
Must have completed 24 hours of basic diabetes education inclusive of diabetes - specific topics, educational principles and behavioral strategies.
Certified Diabetes Care & Education Specialist preferred or CDCES eligible within one year of hire.
Complexity of Work:
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
Must be able to work in a stressful environment and take appropriate action.
Minimum of three to five years experience in medical-surgical nursing or related field.
Previous teaching experience preferred.
Requires full range of motion, manual and finger dexterity and eye-hand coordination.
Requires standing and walking for extensive periods of time.
Requires corrected hearing and vision to normal range.
Requires some exposure to communicable disease or bodily fluids.
Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: when it requires walking or standing to a significant degree; or when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. Note: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.
