Nurse Practitioner/Physician Assistant: Flexible Schedule and Excellent Source forSupplemental Income!

Perform Health Risk Assessments on patients in theirhomes and/or in-clinic.

NPs/PAs with HRA experience preferred

Conduct comprehensive in-clinic or in-home health riskassessment to identify all active and chronic disease conditions as well asdetermine all physical, mental and social needs present at the time of thevisit

Obtain vital signs, measure BMI, reviewpharmacological therapy and conduct a physical examination

Educate and counsel patient and family onany conditions identified during assessment and screening procedures.

Part time and Flexible: Set your own hours, must be availablefor 15-20 weekly hours

Fixed Payment at $110 per in-home screening/$85 in-clinic

Each screening takes 1hr. on average and there is no prescribing

Malpractice Coverage, paid training, scheduling and EMR systemprovided

Requirements:

• Must have active NP/PA state license
• Must have active NP/PA Board Certification
• Must have valid state driver license and automobile insurance
• Knowledge of clinical standards of care
• Ability to complete and clear a background and credentialing screen
