DEPARTMENTAL SUMMARY: The Home Health department provides regional skilled, medical services to patients that are unable to leave their homes to access medical care. Services include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work, aide, and phlebotomy
The Home Health Clerk 2 provides support to office staff; monitor and process documents for Home Health; follows established procedures for accomplishing assigned office functions. This position does not directly manage any other Caregivers
ESSENTIAL FUNCTIONS AND DUTIES:
Create new patient charts.
Coordinate incoming lab results documents, which includes distribution to applicable caregivers, and accurately filing in patient chart.
Responsible for coordinating incoming signed patient admission documents from caregivers, tracking in documents in EMR, and accurately filing in patient charts.
Efficiently and accurately processes all unsigned Home Health orders and Home Health Certifications, sending to provider offices for signature.
Processes all incoming, M.D. signed, Home Health orders and certifications , track into EMR, distribute to applicable caregivers and accurately file in patient chart.
Run Order Report per established guidelines, research and follow up on past due and unresolved orders documents.
Provides coverage on collection and processing of patient discharge documents from caregivers, and monitor discharge audit charts.
Coordinates pulling patient charts and preparing for storage per established guidelines.
Provides effective coverage for primary office roles in the absence of assigned staff as needed.
