Technical Performance – Phlebotomist
Performs venipuncture to obtain blood samples within two attempts from patient of all age groups following all handling policies and procedures.
Assists in basic procedures, report production, specimen procurement, and accessioning with no more than one technical error per 12 month period.
Operates assigned equipment and performs all procedures in a safe manner as instructed

Technical Performance – Patient Access Representative
Accurately and efficiently registers patients by obtaining and verifying pertinent demographic and insurance information while documenting and/or obtain patient signatures on all medical/financial documents as required by FMH, Federal and State requirements.
Performs quality control functions related to patient registration to ensure completeness and accuracy for patient safety to include information obtained during scheduling, pre-registration and/or registration.
Effectively communicates to appropriate parties their financial responsibility, collects patient co-pays, deductibles or deposits at time of service.
