Description:
Providence is calling a Dietitian to work at PACE Redmond at Providence Elderplace in Redmond, Washington.
The Dietitian provides nutrition assessment, counseling, and planning upon inter-disciplinary team referral for participants identified as being at nutritional risk according to established screening protocol or as ordered by physician. Monitor and document the participant's response to mode of nutritional therapy used.
In this position you will:
Provide care and service appropriate to the age of the geriatric participants enrolled in the program
Read and interpret medical charts
Perform nutritional assessments and referrals based on established screening protocols and makeappropriate recommendations
Enter nutritional information in medical charts using approved format and standardized language in dietetics
Talk with participants and/or caregivers, make home visits, and provide individualized nutrition education as needed
Discuss participant nutrition issues with members of the IDT
Qualifications:
Required qualifications for this position include:
Bachelor's Degree in Nutrition -OR- Bachelor's Degreee in any field other than Nutrition, with Didactic Program in Dietetics and Coordinated Program in Dietetics Completed an Accreditation Council for Education in Nutrition and Dietetics accredited supervised practice: Dietetic Internship Program or Individual Supervised Practice Pathway
Current licensure as a Registered Dietitian in the state of Washington
Driver's license valid in Washington state required upon hire
Current auto insurance coverage required upon hire
Preferred qualification for this position include:
Masters Degree in Nutrition or other related field
One year clinical dietitian working with a geriatric population
